 Items that you will find in your assignment book: “15 and Out” form 1 st 9 week schedule  Assignments books must be filled out and carried to all classes.  If you have to leave a class for any reason you must have your assignment book.  If your assignment book is lost, you must purchase a new book.  This is the form that your teachers will use if there is a situation/infraction in the classroom or if you need to leave the room.  This form must be kept in your assignment book.  At the end of the 9 week, any student that has 15 or less marks will be invited to the 9 week incentive.  If a student gets a total of 15 or more marks, he/she will be called into Team to discuss how the issue can be corrected and possible disciplinary actions. 15 AND OUT FORMS
