What We Know-Context Demands and expectations are rising Difficult current and future financial backdrop Change is inevitable- regional, integration with health, legislative (SSWB Wales Act) Political - change at local and national levels Sustainability- the need to change to meet future generations changing aspirations Regulation- improving quality and standards 3

So Today’s Focus is Integrated Assessment… As the cornerstone of social work, OT and nursing assessment and care planning is key Is about changing the mind set, culture and practice of staff and teams Its focus is on strength based, outcome focused care and support planning Implementing parts 3 and 4 for the SSWB (Wales) Act 2014 and we will touch on part 7 4

 Duty for adults and children to be assessed for care and support and for carers to be assessed for support  The aim is to simplify the assessment process  Where appropriate, assessments for a carer and a cared for person can be combined What does this mean?  Single Process of Assessment, greater consistency (common core data set) and proportionate (simple /complex) and portable – should include practitioners from both LA and ABUHB  Person completing assessment has necessary skills and training  Sets out 5 domains for assessment: ◦ Persons Circumstance, Outcomes, Barriers, Risks & Strengths Part 3 – Assessment of individuals-The SSWB (Wales) Act

Responding to identified needs  The Act will set out an eligibility framework creating an enforceable right for the individual  A care and support plan must identify the personal outcomes and set out the best way to achieve them  There will be a continuation of direct payments plus extended availability to other forms of care and support What does this mean?  Move from ’What is wrong?’ to ‘How can I help?’  Introduces a ‘Can, and Can Only’ Test – National Threshold  Eligibility Criteria: ◦ Physical or Mental Ill Health, Age, Disability, Dependence on Alcohol or Drugs and other similar circumstances  LA in partnership must produce a Care and Support Plan. Format agreed by LA and ABUHB  Direct Payments must be made available where they can meet needs Part 4 – Meeting Needs-SSWB (Wales) Act

Eligibility determination is based on a judgement which flows from a process of assessment of needs structured around these 5 elements: Circumstances Personal Well-being Outcomes Strengths and Capabilities Risks Barriers Eligibility status is given to the need – not the person 8

We Have Made a Good Start Work well together in the region- partnership board in place, examples of good regional working – safeguarding, frailty Lead Well- at political and officer levels Designed structures to enable users and carers to have a voice Have skilled, experienced staff in place Deliver training and support and link with WD regional training plan 10

Current Regional Position (1) ABUHB- Lead Senior Nurse identified and additional nurse manager support planned B Gwent- tools embedded in IT system-will test out within joint teams, work commenced with staff on critical reflection and underpinning knowledge and skills. Also discussions commenced with service providers to move to outcome focused commissioning Caerphilly- tools embedded in IT system and STAR team implementing, intention is to roll out to all teams in Borough. Conversation to commence with commissioners and providers of services 11

Current Regional Position (2) Monmouthshire tool developed and imbedded in bespoke IT system. Intensive support and training for staff provided. Outcome measurements being captured and changes to commissioning and provision of services commenced Newport- tool being tested in pilot area to test before rolling out to all teams. Form imported into IT system Torfaen- tool developed and changed to meet local need and embedded in IT system. LA team of various roles identified to test out tool and be ‘champions’ for new change in practice 12

Challenges Structural change at the highest level and ownership by leaders is essential Define and be clear about what we mean by integration Ensure our partners are alongside us Relook at how we commission services to meet future need and demand Ensure that we inform and communicate with our communities Deliver at scale and pace 13
