Slide 3 Submission Requirements HSAG requested the following information: –2011 HEDIS Roadmap –2011 Final Audit Report –2011 Final locked IDSS file (.csv and.xls format) For those MCOs who do not report to NCQA, submission of the 2011 AHCA Reporting Template –Supplemental Questionnaire: HMOs/PSNs If you have both a reform and non-reform contract and the process to collect and calculate the performance measures are the same, you may submit one document and indicate that the submissions addresses both contracts.
