Purpose of the CHART 1.Convene stakeholders from multiple sectors that are committed to promoting community health 2.Assess & monitor status of health for your local community(i.e., Physical Activity/Nutrition/Tobacco-PANT) 3.Develop a CAP (Community Action Plan) to promote physical activity, good nutrition, and tobacco cessation 4.Implement strategies which achieve increased physical activity and good nutrition 5.Use the CHANGE tool to evaluate and gather data on the impact of your ACHIEVE efforts 6.Develop and implement strategies to sustain the movement
