Health Plan Finder to WA Conn Assisting customer through HPF and continue to WaCon. IPA with an “-” in their username prevents data to transfer from HPF to WaCon. (This will be fix mid January 2015) Start DSHS application process – just need 5 information: HoH full legal name, DOB, SSN#, address and services applying HPF WaCon
What’s NEXT? Customer can contact Customer Contact Center OR Come to any Community Services Office between 8am - 2pm for same day interview Supplemental information/verification may be requested – pending letter in the mail on what needs to be verified before benefits can be approved
