7 Qualifying Insurance Premiums
Health, Dental, Vision, and Long- Term Care Insurance Medicare Part B, Part C, and Part D and Medicare Supplements Individual Policies on and off the Exchange Employer-Sponsored Group Health Coverage (as long as not paid with pre-tax payroll deductions)

9 Claim Submission Options
ONLINE MAIL Submit claim securely online through MyMidAmerica.com claim form to Mail claim form to: P.O. Box Lakeland, FL 33802

10 Accessing Your Online Account
Go to Select Access Account. If logging in for the first time, use your SSN as your Username and the last four digits of your SSN as your password. You will then be prompted to update your login credentials. If you have logged in before, simply log in with your established credentials.

12 Online Claims Submission
Go to Select Submit Claims Next, select Submit A One-Time Reimbursement Request Upload documentation 7-10 business days for reimbursement $2.50 fee for each reimbursement, capped at six distribution fees per year

Claim Form Include receipts or other documentation Mail or fax 7-10 business days for reimbursement $5 fee for each reimbursement, capped at six distribution fees per year

Premium Notice from your insurance company that includes: Premium Amount Effective Date of Coverage Name of insured – you, your spouse, or another qualifying dependent Insurance Premiums must be paid before reimbursement Recurring premium reimbursements remain in effect for the earlier of 12 months or when your premium changes* *New claim form or online submission with required documentation must be submitted to renew recurring claim after 12 months

Statement in writing Confirms policy is still in effect Allows us to continue recurring reimbursements Monthly vs. 12-Month Attestation Monthly Attestation Required if participant is enrolled in individual insurance policy Submitted via Attestation Form Request Monthly Attestation Reminder when submitting claims online We send you a monthly – you attest by clicking a button in the 12-Month Attestation Allowed if participant is not enrolled in individual insurance policy Submitted via Attestation Section of the claim form
