Upload Documents Appears when documents are requested Badge icon (#) indicates number of documents requested View Messages Link to view account messages Badge icon (#) indicates number of unread messages Application Enrollment with ID# Ability to view household names and enrollment detail (for submitted applications) Link to View Details Arrow will allow customer to choose which application to view details on (if there are multiple applications) Trainer Talking Points: Customers have the opportunity to upload documents, view account messages or view their household enrollment details when they first open their Mobile Application To upload documents requested customer clicks Upload Documents – this will only display if a customer has documentation requested on their application. This means they have likely been determined conditionally eligible and need to provide documentation to prove their conditional eligibility within 95 days. If documentation is not received within 95 days, customers may be termed. - Termination of coverage is limited to: not providing proof of SSN, citizenship, lawful presence and Medicare and Incarceration - The following result in a loss of APTC: not providing proof of income, no other insurance (Medicare, Peace Corps, Veterans Insurance & Tricare) - Failure to provide proof of American Indian/Alaska Native documentation may result in loss of cost sharing reductions and ability to report a SEP once a month to change their plan To view messages customer clicks View Messages To view enrollment details customer clicks View Details under customer names - In the Mobile App the user will not see any Coverage or Plan info though until they have submitted an app 11/10/2018

Document Center Upload documents Find In-Person Help Search for brokers/navigators Help Center FAQ’s Contact us App tutorial My Profile Notification preferences Change password Update security questions Update address Trainer Talking Points: Any changes made to account settings through the Mobile Application will save to the customers account through the Web Application accessed at The Application menu is available to individuals who have not logged in – this would display FAQ’s, Contact Us and Application Tutorial When customers click the top left menu customers they will see a drop down of application options: - Home will take customers back to the home screen - Document Center will take the customer to the document center - Help Center will take the customer to FAQ’s, Contact info and App Tutorial - My Profile will take the customer to update their notification preferences or change password 11/10/2018

Field level help Trainer Talking Points: When in the document center you can click the ? (question) to see a pop-out of what is considered acceptable documentation To upload the customer clicks Upload If a customer needs to upload documentation that is not being requested they can click Upload a different document To see a list of submitted documentation click Submitted 11/10/2018

You may see the following statuses: No documents submitted Will be displayed if no documents are submitted Pending Review Will be displayed if there are documents pending review Invalid Documents Reason for invalid document will display Reviewed Documents Documents that have been reviewed Trainer Talking Points: When in the Submitted Document option you will see any pending/invalid or reviewed documents Invalid Documents – Documents that were not valid to provide proof of whatever is being asked Pending Review – Documents that have been uploaded and are pending Exchange review Reviewed Documents – Documents that have bene reviewed and approved as acceptable to prove conditional eligibility 11/10/2018

Tips for taking a photo Take a Photo Goes to active camera Pick from Gallery Goes to phone camera roll Option to select 1 photo at a time to upload Trainer Talking Points: When uploading documents customers will see the option to Take a Photo with their phones native camera functionality and Pick from Gallery to select a photo already taken in their phones gallery Customers will see photo taking tips and have the option to go back (< white back arrow) Customers will upload one photo at a time for whatever documentation is being requested – if a customer uploads multiple pages it will collate and upload as one PDF 11/10/2018

Take a photo use your devices photo button Customer cannot select documents to upload – only photos Customer will be prompted to: Submit or Retake photo Once Submit is clicked customer will be prompted to confirm they want to submit Yes or No Trainer Talking Points: 11/10/2018

Once submit is clicked this page will display Click Yes, add another page or No, I’m done to complete upload Uploading another page will allow the customer to upload another page and will collate any uploaded documents when a customer clicks Submit Another Page 11/10/2018

Once a customer submits their document they will see a confirmation screen They will see the option to: Submit Another Page Upload Another Page or View Submitted Documents Trainer Talking Points: 11/10/2018
