Practice Perspective As a GP practice: Identify patients earlier Staff education and understanding Timely conversations with patients, giving people the time to plan and discuss options Awareness of available support Help avoid crisis and loss of independence where possible

Local Solutions In 2014/15 - Improving Access to Psychological Therapies (IAPT) enhanced service pilot for patients with Long Term Conditions. Help patients and carers prevent avoidable hospital admissions. In 2015/16 - Expanded to include people with dementia – working to reduce anxiety, low mood and stress within the household. Across 2014/15 - 6% of all referrals to the IAPT pilot were for people with dementia. By month 5 in 2015/ % of all referrals were for patients with dementia

Role of the GP To consider the whole person, including other chronic and long term conditions and the potential impact of medication To weigh up the pros and cons that an intervention may have on a patient- communicate these Recognise carer stress- physical and mental Encourage patients and family to accept help Refer and signpost to post diagnostic support
