Student Responses: Students are required to submit an explanation, in their own words, of why they have struggled to maintain an acceptable GPA This allows for students to reflect upon their own decisions as well as to address problems within the appointment Obstacles/Barriers

Proposed Study Plan with list of specifications/changes: Student Responses: Student explains their own plan to make improvements Ensures students are prepared and have goals in mind before the meeting

The College of Engineering expects that given this “second chance” at success, you will attain the required minimum GPA listed to return to good standing and maintain good standing through graduation. Verifying the motive of the appointment University Polices-Desired Results:

The College of Engineering expects that you will utilize the various resources including Instructors, TAs, Tutoring program for lower level engineering course assistance and consult regularly with your Faculty Advisor. → Engineering Tutoring → Campus Tutoring → TA Office Hours → Professor Office Hours University Policies-Resources:

Resources-Engineering Tutoring Center: The Tutoring Center is equipped with students to teach all core Engineering courses, as well as offer review sessions before exams. It is open five days a week for three hours

Resources-University-wide Tutoring: The University offers tutoring: For many common classes On a one-on-one basis Through Supplemental Instruction-one semester course designed for classes like chemistry, sociology, etc.

Monitor yourself against a minimum grade of C or better on all feedback from class: homework, quizzes, labs, exams, papers, etc. Showing students who is responsible for bettering their grades University Policies-Accountability:

Positive: improve grades, GPA and return to good standing. Negative: Dismissal from the College and the University. Your registration will be restricted for one calendar year at The University of Iowa. University Policies-Consequences:

Signatures: This signature shows that the student agrees to all of the University policies and confirms their proposed plans This is then scanned in to the students file
