Ashley H. The pros to having a transition plan is it helps you focus on what you’re doing. It also helps you realize what you have and more about yourself just in case you forgot. It also helps you know more and help others know what to do if they need a transition plan. The cons are it won’t help you if you don’t check it every once in a while to refresh or add more to keep you organized. Youth in Transition

Isaiah R. [The Transition Plan] was helpful as well as eye-opening. It gave me a better idea of responsibilities I have and holding me accountable and knowing specific details. But on the other hand, it did ask a lot of personal questions and redundant questions which was not useful. Youth in Transition

Richard W. The Transition Plan was very helpful in organizing and categorizing various professional and important information, but at the same time I felt as though some of the information that was required to complete was a bit inappropriate. Some of the questions asked I felt were a bit more personal and irrelevant, which resulted to leaving some spaces blank, but for the most part I thought it was a great organizational tool. Rasheed F. The transition plan was very straight forward and clear. It was [also] redundant. A lot of questions were unnecessary questions such as relationships, “favorite band”, etc. It is nice that it has sections and all the information is saved in one place. Youth Mentor/College Student

Shy’la S. I would utilize this for food assistance. I would also use it to save all my info. I could also use it to get connections in my community. If I didn’t have internet access it would be hard to get my information. It takes up time to do the transition plan. (It’s) a lot of detail needed in the transition plan you might not have known. Hannah S. The transition plan was very helpful because it made me think and look deeper to who (I) really am. And it was helpful because it lets me know my plan and helps me to look at the things I’m planning on doing in the future. Youth in Transition

Jerome G. [The Transition Plan] is an easy, effective way to keep track of information. It provides good contact information for resources and allows time for self-reflection.
