History Elementary school: Plains Middle school: JFH High school: BHS I attend BHS. I am in 9th grade. My favorite class is PE because we play basketball. My least favorite class is Spanish because it is hard.

Strengths I am good at soccer and basketball. I enjoy playing basketball. I like to use my hands when I do projects.

Friends and Hobbies My best friend is Chris. In my free time I play with my RC truck and PS2/PS3. I play PS3 with Josh. We both enjoy shooting people.

Parent/Student Comments and Concerns I am worried about failing Spanish.

How My Disability Affects Me in the Classroom My disability affect me in class because of my hand writing, I don’t study for test and I for get to turn my homework in on time.

Effects of my disability My disability effects my learning in: –Spanish –Handwriting –Study for test –Turning home work in time

Social & Emotional I get along with my classmates because most of them are my friends. I don’t like school. I get along with my teachers and the school staff great because I like them.

Transition I want to go to college. I am going to try to pass all my classes. I plan to save up money. I want to get a car when I turn 16.

Employment I work for Lohr construction. I have done that for 2 or 3 years. I enjoy my job because I work with my dad and I get to buy whatever I want.

Keep passing grades in my classes.

Participate in 4 out of 5 leisure activities.

Participate in a field trip to MTC.

Goals and Objectives Measurable Annual Goals Pass all my classes. Raise my hand and ask for help when I need it. Bring all my necessary materials including homework to class 70% of the time. Practice using a word processor for long written assignments.

Accommodations The accommodations that I have on my IEP are: –Preferential seating –Use word processor when I have to write something longer then two paragraphs –Extended time on test and quizzes I have these accommodations because I have bad handwriting and need help on test and quizzes
