Care closer to home We have an ageing population and increased complexity in providing health care. Match this with a move to manage people closer to home (including avoidance of admission and facilitation of early discharge) and the level of complexity and acuity in the community is increasing. This places an increased demand on what is seen as a traditional role of the community nurse where nurses are now needing increasingly advanced skills to manage patients effectively. The community is becoming an increasingly challenging and rewarding place to work…..

Southern Health in Context

Southern Health Provides services across Hampshire, Dorset, Wiltshire, Oxfordshire and Buckinghamshire Employs over 7000 staff based at over 150 sites We serve a population of 1.3 million people In year to March 2014 we supported 251,000 individuals making 1.2 million community visits We are now going through a transition where Older Peoples Mental Health and Community teams are merging The five year forward plan is also allowing us to work more closely with other community providers to develop services

With this comes challenge innovation and opportunity that we would love you to be part of.

Preceptorship We have a profession and field specific preceptorship programme for all newly qualified staff. Runs 4 times a year so you will be able to take up a place within 3 months of starting. Led by practice educators who are from the same field they support – giving them a greater insight into what your needs are. Better supports the transition from student to registrant in a safe way.
