Magnets- A Tool for Coordination with hopes of future Collaboration

HVN Mission Statement  To assure an efficient health care and social support delivery system to families by optimizing collaborations and maximizing resources among existing maternal and child health programs. 

Maximize Available Resources  look for opportunities to increase the efficiency and effectiveness

Our Community Our Families  Communities and families need an array of services to address the needs of young children and their families.  Linkages are essential to better meet the needs of the most vulnerable

Legal and Ethical Issues  Can case managers to work together effectively?  What if case management agencies operate under different philosophies and mandates that seem at odds with each other.  HIPAA (Health Insurance Portability and Accountability Act)  Must follow your agency’s policy for release of information

We do not want our Clients Confused  Some clients become con­fused about how the system works and frustrated by the fact that it requires so much effort and time.  As a result, some clients become detached from systems of care while others receive the same services repeatedly as they are juggled between case managers who concentrate on what they are able to provide, rather than what clients need.  While the absence of case management can hamper client access to needed services, the existence of multiple case managers working in an uncoordinated system can contribute to the fragmented service delivery that case manage­ ment is meant to alleviate.

Coordination  an impor­tant strategy for improving interagency communication and promoting more efficient delivery of case management ser­vices to families.  Coordination can help lay the groundwork for future collaboration.
