Personalisation in Leicestershire

Transformation of Social Care Putting People First In 2007 the Government created a concordat, an agreement between departments, which outlines the major changes across all social care and health services in the country. The agreement sets out to transform all public services, to give the people who use those services greater choice and more freedom to do the things they would like to. In Leicestershire Adult Social Care, we call this vision Promoting Independence, Changing Lives.

Personalisation Our Purpose: To enable adults in Leicestershire to be as independent as possible and play a full and active part in the life of their communities.

Personalisation Universal Services Prevention & Early Intervention Social Capital Choice & Control At the Local Authority level, Putting People First is known as Personalisation. This involves four key elements -
