Early Intervention Mock Evaluation

The following is a mock evaluation of a 15 month year old boy. The boy in this evaluation will be named “Tom” and the mock team will consist of a service coordinator, speech and language therapist and a social worker. They will administer the Mullen Scales of Early Learning and The Battelle Developmental Inventory.

Step One The evaluation team will meet and discuss the child and what test would be best to administer during the assessment Discuss the referral and what the service coordinator observed during the initial meeting

Step Two Arrive at the family’s home Meet child and parent/s Introduce each team member Explain to family how the test works and what the team is looking for during the evaluation Begin the Mullen Inventory Scale and Battelle

Gross Motor Purpose: to assess how the child moves different body parts and how he or she can move across a room Testing (13-20 month yr old): Can sit with arms free, pulls self to stand, gets from sitting to hands and knees, walks with one hand held, stands alone (1-2 seconds), walks alone (4-5 steps), throws a ball underhand MOCK EVALUATION: -Tom is placed on the floor by mother. The social worker observes Tom sitting. The SW then requests Tom to stand up. The SW then asks Tom to walk over to her. The SW asks the service coordinator to throw a ball to Tom and observes to see if Tom throws the ball back and how he throws it. -Tom is able to attend to all these tasks, he scores in the average range in this section.

Fine Motor Purpose: to assess the child’s hand-eye coordination Testing (13-20 month yr old): takes blocks out, puts blocks in, uses two hands together, turn pages in a book, imitates crayon lines (horizontal, vertical, any direction) MOCK EVALUATION: -The speech and language therapist takes a bag of blocks and places it in front of Tom. She then asks Tom to take the blocks out of the bag and place them back in. The S & L therapist claps, and observes to see if Tom imitates her. She then takes a book and begins to read it to Tom. She asks Tom to turn each page. The S & L therapist then takes a blank piece of paper and draws a vertical line, horizontal line and circle and asks Tom to do the same. -Tom attends to all these tasks and scores in the average range for this section

Receptive Communication Purpose: To assess how much a child understands words and requests Testing (13-20 month yr old): Recognizes familiar names, words, and own name, understands simple verbal input (wave), understands gestures and commands, gives toy on verbal request, identifies objects MOCK EVALUATION -The speech and language therapist asks Tom to point to his mother. She takes a mirror and asks Tom to point to himself. The S & L therapist takes a toy car and places it in front of Tom. She lets him play with it for a while and then asks him to give it back to her. She takes out several blocks and asks him to clean up after stacking them for a minute. She also takes out a book and asks Tom to point to several different animals. -Tom was able to attend to most of the tasks but was not able to identify the different animals. He still scored in the average range for this section.

Expressive Communication Purpose: to assess if a child is able to make their needs and wants known Testing (13-20 month yr old): vocalizes two- syllable sounds, plays gestures/language game, says up to 7 words, combines gestures and words MOCK EVALUATION -The Speech and Language therapist observed Tom’s language throughout the evaluation. She asked him to point to particular objects. She also a car, a ball, a shoe, a baby doll, a toy puppy, a toy fish and a book and asked Tom to repeat the object names after her. -Tom was not able to imitate words and had trouble vocalizing different sounds. He scored below average in this section.

Cognition Purpose: to assess a child’s problem solving skills and how he or she thinks Testing (13-20 month yr old): looks for ring hidden under washcloth, turns cup right side up, makes object association, looks for car under two washcloths, shows interest in book as hinge, attends to picture, looks for toy covered then displaced MOCK EVALUATION -The speech and language therapist took a ring and placed it under a white cloth and asked Tom to find the ring. She also placed a cup in front of Tom and asked him to drink from it. The S & L therapist took out a puzzle that only was missing a circle and square and asked Tom to place the shapes into the right place. The S & L therapist showed Tom a picture of a child and asked him to point to the different body parts. The S & L therapist also took a toy car, showed Tom that it was in her hands and then closed her hands and asked Tom which hand the car was in. -Tom attended to must of the tasks, had difficulty with labeling the body parts. He scored in the average range for this section.

Social Emotional Purpose: To assess how a child interacts with others Testing (13-20 month yr old): expresses displeasure for or dislike of certain activities, expresses enjoyment of or preference for certain activities or situations, plays peek-a- boo, discriminates between familiar and unfamiliar people, shows appropriate signs of separation anxiety when removed from his or her parent or caregiver MOCK EVALUATION -The social worker gave Tom two activities to choose from to observe if he has a preference and can attend to that task. The social worker gives Tom four cups and shows him how to nest them into one another. Then after Tom imitates her and completes it the team praises him to see his reaction to the praise. The team evaluates how the child reacts when they first meet him, along with asking the parent or guardian these particular questions. The social worker asks Tom’s mother if he screams and cries when he is dropped off at daycare. -Tom seem to become very anxious around strangers. He also did not seem to acknowledge the praise from the team members. Mother reports a tough time getting him to stop crying at daycare after she leaves. Tom scored below average in this section.

Adaptive Purpose: To assess how a child is learning to take care of themselves Testing (13-20 month yr old): Feeds himself or herself bite size food, drinks from cup with assistance, chews food using a rotary jaw movement MOCK EVALUATON -The social worker asks Tom’s mother if he is able to complete these tasks under testing. -Tom’s mother reports that Tom is able to feed himself and attend these tasks. Tom scores in the average range for this section.

Outcome Due to Tom scoring below average in the social emotional section as well as the expressive communication section he will be eligible for Early Intervention The service coordinator will make a summary of the assessment and meet with the family to create a individualized family service plan.
