Huntsman Site Specific Training By: Larry Medina

Daily Sign In / Sign Out Sheet Every employee working in the Huntsman Odessa Complex, must use the Sign In/Sign Out Sheet. You must sign in every morning and sign out every afternoon as you leave work. The Sign In sheet asks “Did you have an employment or job related accident or injury today?” You answer the question by the initialing in the “Yes” or “No” column. This is a zero tolerance policy and will be enforced.

Accidents and Incidents All accidents, injuries, near misses and any other incidents must be reported to your supervisor immediately! Huntsman houses a qualified Medical Department available M-F 7:00-5:30. Certified personnel are on call 24/7 and can be reached through the East Gate. Minor injuries must be taken care of before they become more serious.

Emergency Evacuation In the event of an emergency: ► Stay Calm ► Listen to the alarm for instructions. ► Monitor wind direction ► Stay up wind of any release! ► Walk to the nearest “Evacuation Point.”

Accounting for Everyone ► Do not drive to evacuation point. ► Turn off vehicles and any other equipment on your way out. ► Upon arrival to the evacuation point, call your supervisor on the plant radio. (Radio Ch. CNT-1) ► This is very important!

Work Permits Before starting any job, obtain a work permit. Hot Work- Covers any job which includes the potential for sparks. Welding, grinding, buffing, chipping, the use of pneumatic tools, and the use of any motorized equipment including vehicles. Start Work- Covers any job that does not involve hot work. Confined Space Permit- Must be issued before entry into a confined space. Supervisor must sign off on permit.

Motorized Equipment  Only certified personnel are allowed to use heavy equipment such as Manlifts, Cranes and Forklifts.  A daily inspection must be made before using any piece of heavy.  All equipment must be listed on the work permit and a Huntsman operator must escort the equipment before entering any process area.

Monthly Color Code  All electrical tools must be inspected on a monthly basis and this should be verified by a colored tape representing that month.  All electrical tools require the use of GFCI.  The monthly color code applies to personal fall protection and chokers also.  The color brown in this system is equivalent to a red tag.  The presence of the monthly color does not substitute for the daily inspection.

Scaffolds All scaffolds must have a green tag or a red tag. Green tags with the current day signify that the scaffold is safe to use. You must read the tag for warnings and tie off requirements. Red tags signify that the scaffold is unsafe to use. Incomplete scaffolds must be red tagged until complete.

Eye Protection  Safety glasses are required upon entering the gates in the complex.  Double eye protection is required when grinding and buffing.  This consist of matrix face shield and goggles.  Different styles of goggles are available.

Cell Phones  Cell phones are NOT allowed inside process areas.  Keep items such as cell phones, pagers etc. in your lunch boxes in the break areas.  This rule will be enforced and violators will be reprimanded.

Hair Policy  All employees, visitors and contractors are required to follow the hair policy.  Mustache can only extend to corners of mouth.  Side burns can only extend to the bottom of the ear and can not be flared out.  Goatees and beards are not allowed.

Fire Retardant Clothing (FRC’s)  All employees entering any process areas are required to wear frc’s.  Frc’s must be worn correctly with sleeves rolled all the way down and front side zipped up all the way.  All jackets must also be frc or must be worn under your frc coveralls.
