Our story when a problem was reported in one of our homes… Safeguarding alerts All the information on the provider brought together Meeting with different agencies Assessed the risk to residents

Outcome More issues raised Medication Care plans and records Staff matters Health and safety Action plan

Fire and Rescue Service asked to visit Pharmacy assessment Nursing care All residents reviewed Talked to families Contract notice

Whilst this was happening people were kept safe Care Trust Plus staff visited twice a day at different times Emergency Duty Team at other times Pharmacists checked medication

All residents assessed CQC visit Home not fit for purpose Residents found new placements of their choice some in supported living Quality team met some of the residents in new placements All are well and happy
