2. What is MOCH programme
NHS England found that care home residents don’t have the same access to a pharmacist as patients in their own home.
MOCH allows specialist pharmacists to provide support for care home residents by working as part of the care team.

3. What we aim to achieve
Enhance the quality of care through better medicine use
Reduce the risk of harm from medicines and support safer medicines systems
Release resources through medicines optimisation and waste reduction, reduction in hospital admissions and release of carer’s time.
