 Ideas on how QIT team members can work to integrate CCFC into their intake process  Suggestions for intake staff to consider before they begin to talk to consumers about CCFC and natural supports  A list of the “Whys” someone can benefit from a support system (to assist with conversations with consumers)  Questions to ask participants to identify possible support people  Open Discussion:  Would like to hear from programs that are integrating CCFC into the intake process  Questions from programs that are thinking about integrating CCFC into the intake process

Encourage administrators to take part in the entire planning stage of developing the intake process to include CCFC Involve the entire program staff in the planning stages Try to use an open discussion format and hear all of staff members’ opinions

Together as a program create the intake format and forms you want to use Set a date to begin Plan on having follow up meetings to check in on how it is rolling out (we recommend every 2 weeks initially) Make changes as necessary

It is important… To be mindful of the frame of mind your client is in when asking them about involving others in their recovery That you have set aside enough time to spend talking about involving others in the client’s recovery To ask open-ended questions to avoid yes or no answers To remember you want this to be a conversation with the client about the importance of a support system in one’s life

Research shows that treatment is more effective and recovery is more likely when an individual has support from people that are important in their lives. Having a strong social support system is one of the best ways for a person to maintain a healthy and positive frame of mind. A strong social support system can alleviate stress by providing individuals with another person to share their problems with.

A support person may be able to assist an individual with overcoming some of life’s challenges. Being part of a strong social support system can make an individual feel loved and important. Feeling valued by other people can increase an individual’s self confidence and self esteem.

Individuals can call on members of their social support system when they want to participate in an activity or just talk about what is going on in their lives. Without social supports many people are more likely to feel lonely, isolated, and bored.

Who is actively involved in your life today? Who do you turn to when you need help or support outside of your program? What person comes to mind when you think of someone that encourages you to do well? List some people that are special in your life. Who we will be involving as your support system while we are working together? What would it be like for you if someone in your personal life understood what you’re going through, and knew the best way(s) to help? Who comes to mind that you wish could be this support person?
