Below is an overview of incentives associated with the 2009 SLPS Wellness Plan Avoid $40 per month medical premium contribution $75 gift card for completion of the Health Assessment. Funded by the UHC wellness program. $25 gift card for completion of online Health Coaching program. Funded by the UHC wellness program. $75 gift card for completion of the telephonic Health Coaching program. Funded by the UHC wellness program. Step 1 (Screenings) There were over 65 SLPS screening sites As of May 19, 2009: 3,488 Active employees participated 96.8% participation (assuming 3,602 total eligible active employees) Step 2 (Assessments) Online Assessments were due by May 18, 2009 and paper copies had to be post marked by May 18, 2009 As of June 6, 2009: 3,181 Active employees participated 88.3% participation (assuming 3602 total eligible active employees) Combined Participation (Wellness Plan Compliant) 88.3% assuming those who completed the Assessment also completed the screening Final participation to be determined mid-June /14/20145 St. Louis Public Schools Holmes Murphy & Associates – Benefits and Risk Advisors

SLPS placed heavy emphasis on communicating the wellness plan in order to obtain the highest level of participation and avoid charging employees the penalty for non-compliance. SLPS Human Resources, UnitedHealthcare, and Holmes Murphy met with the school nurses to explain the plan and request support on January 29 and 30, /14/20146 St. Louis Public Schools Holmes Murphy & Associates – Benefits and Risk Advisors

Communications sent to employees: 1) February 2, 2009 – Announcement Letter and Tri-Fold Overview with Screening Dates mailed to employees homes by UnitedHealthcare sent via blast by SLPS 2) February 13, 2009 – SLPS blast with Q&A 3) March 31, 2009 – SLPS blast reminder with Step 2 Announcement Memo, updated Q&A, and several other flyers 4) April 30, 2009 – SLPS blast reminder with Step 2 Memo 5) May 14, 2009 – SLPS blast and voice mail blast with updated participation stats and reminder of last date to complete Assessment (5/18/09) Also posted all SLPS Wellness Plan Communications and material on the intranet site and placed posters up in schools. 2/14/ St. Louis Public Schools Holmes Murphy & Associates – Benefits and Risk Advisors
