1. Action Is Required for Premium Reward
Are you enrolled in COVA Care or COVA HealthAware? If so, you are eligible for a reduction in your monthly premium.
You and your covered spouse must complete or update a health assessment online or by phone.
Have your physician complete and submit your biometric screening results from 7/1/2015 or later.
Reductions will be effective the following month after submission of BOTH requirements.
$17 month reduction for employee ($204 annual savings) and/or $17 month reduction for covered spouse ($408 savings annually)
To earn a Premium Reward effective July 1, 2016, you must be enrolled in COVA Care or COVA HealthAware.
Between July 1, 2015 and June 30, 2016, you need to :

You also need to report your biometric screening results. Measurements taken as far back as July 1, 2015 will qualify. If you need to report results, download a Physician Form from MyActiveHealth and submit them by June 30. If you are not sure you have submitted your results, contact your Benefits Administrator.
An employee or spouse who is not enrolled in a state health plan will need to call the toll-free number to complete the healthy actions.
Remember, if you are eligible you can complete the requirements at any time! The reward will be effective the first of the month after you complete both requirements and will continue through June 30, 2017.
