Any deviation from this standard must be reported to DOH and approved by DOH Three-Party Initial Contract Agreement and Final Inspection Agreement – DOH recommends one between the municipality/building inspector/consultant, homeowner, and contractor. If you plan to do streets and sidewalks, you should be aware that the block group data may not be available by the time you are working on your application. If you plan to do a survey, you must get permission from DOH prior to doing the survey Survey Methodology handbook that outlines the survey requirements is posted on the Web site in a booklet format for your review.
