TOUCH Initiative Transforming Our Community’s Health The only Florida initiative to receive a Community Transformation Grant from the U.S. Department of Health and Human Services’ (HHS’s) Center for Disease Control and Prevention (CDC). Funded under the Affordable Care Act to support public health efforts to reduce chronic diseases, promote healthier lifestyles, reduce health disparities, and control health care spending. Award of $1.76 million a year for five (5) years ( ). A collaborative effort among more than 20 community organization partners and 10 coalitions coordinated by Broward regional Health Planning Council. Four main focus areas: Tobacco Free Living Active Living and Healthy Eating High Quality Clinical and Preventive Services Healthy and Safe Physical Environment

Tobacco Free Living Goal: To increase the number of places in Broward County where residents are not exposed to tobacco and second hand smoke. Focus Areas: Tobacco Free Campuses Tobacco Free Multi-Unit Housing Tobacco Free Parks

Active Living and Healthy Eating Goal: To help ensure all Broward County residents, especially children, low income, and those living in high-need communities, have access to physical activity opportunities and healthy foods. Focus Areas: Baby Friendly Hospitals Improve nutrition, physical activity, and screen time standards in Early Childcare Centers SPARK After School Fitness Program Improving Nutrition in Public Schools Joint Use Agreements Increasing access to healthy foods

High Quality Clinical and Preventive Services Goal: Encourage individuals and their health providers to engage in preventive health services. Focus Areas: Disease self-management training programs Implementing the patient centered medical home model and chronic disease panel management

Healthy and Safe Physical Environment Goal: Create healthy and safe places for Broward residents that are supportive of an active lifestyle. Focus Areas: Establishing Complete Streets and Smart Growth community design standards to make the streets safe for all users including those who walk, ride bikes, and use public transportation.
