2  Demonstrate the use of evidence-based programs at the local level  Identify interventions and address the core elements of the 4 domains:  - Epidemiology, Surveillance and Evaluation  - Environmental Strategies  - Health System Interventions  - Community-Clinical Linkages

3  Statewide implementation of evidence-based strategies of the NJ Comprehensive Cancer Control Plan  In accordance with Healthy People NJ 2020, address the state’s cancer burden and improve health outcomes for people with, or at risk for, cancer and other chronic diseases  Implementation of Policy, Environmental, and System (PES) improvement strategies that support and reinforce healthful behaviors and lower the risk for multiple diseases simultaneously, including the prevention and control of cancer

4  Engage new stakeholders and affirm commitment of members  Facilitate community-clinical linkages  Implement consistent, evidence/practice based community-clinical interventions  Engage chronic disease partners to implement effective PES changes  Secure the participation of other organizations (e.g., Shaping NJ, NJPN, NJSNAP)  Coordinate with local health departments   Coordinate efforts with the New Jersey Cancer Early Education and Detection program (NJCEED) and the Office of Tobacco

7  Refer community members to NJCEED and FQHC clinics  Outreach and education for Cervical Cancer, Heart Disease, Colorectal Cancer, Breast Cancer including Survivorship, Ovarian cancer, Prostate cancer  Support Coalition Members  Community Health Improvement Plans  Needs Assessment workgroups

8  NJSNAPEd  State workgroups  North Jersey Health Collaborative  Healthier Somerset  Office of Aging  Local Health Departments
