Health and Wellness for all Arizonans azdhs.gov Pediatric to Adult Healthcare Transition Transition is a period of learning, skill building and goal setting experienced by young people as they prepare to move from adolescence to adulthood. A successful transition from pediatric to adult healthcare systems is an essential part of maintaining good health for young adults with special health care needs Children with a chronic physical, developmental, behavioral, or emotional condition, and who require health and related services beyond those required by children generally.* 3 *Defined by the U.S. Department of Health and Human Services, Health Resources and Services Administration, Maternal and Child Health Bureau (MCHB)

The goal of transition is to move young adults toward self-determination by increasing, to the extent possible, decision making and self-direction skills in all areas of life. 4

Medical home is an approach to providing primary care that is accessible, continuous, comprehensive, family-centered, coordinated, compassionate and culturally effective –Include young adults in decision-making at the earliest age possible
