Health and Wellness for all Arizonans azdhs.gov The Office for Children with Special Health Care Needs is a state level partner in the Arizona Community of Practice on Transition. Enhanced our ability to incorporate health as an important aspect of transition planning.

Health and Wellness for all Arizonans azdhs.gov Pediatric to Adult Healthcare Transition Transition is a period of learning, skill building and goal setting experienced by young people as they prepare to move from adolescence to adulthood. A successful transition from pediatric to adult healthcare systems is an essential part of maintaining good health for young adults with special health care needs Children with a chronic physical, developmental, behavioral, or emotional condition, and who require health and related services beyond those required by children generally.* 3 *Defined by the U.S. Department of Health and Human Services, Health Resources and Services Administration, Maternal and Child Health Bureau (MCHB)

Health and Wellness for all Arizonans azdhs.gov Pediatric to Adult Healthcare Transition Good health and well-being are necessary for young adults with special health care needs as they seek employment, education, volunteering, social relationships, recreation and community life. The goal of transition is to move young adults toward self-determination by increasing, to the extent possible, decision making and self-direction skills in all areas of life. 4

Health and Wellness for all Arizonans azdhs.gov Role of OCSHCN in Pediatric to Adult Healthcare Transition Best practices for the healthcare transition of youth with special health care needs –Begin at age 14, young adult, family and healthcare providers jointly develop a written healthcare transition plan; review and revise at least annually –Ongoing assessment of transition readiness and progress –Provision of appropriate transition services, support and planning

Health and Wellness for all Arizonans azdhs.gov Role of OCSHCN in Pediatric to Adult Healthcare Transition Best practices for the healthcare transition of youth with special health care needs –Identification of and coordination with, an adult medical home, sometime between 18 th and 21 st birthday. Medical home is an approach to providing primary care that is accessible, continuous, comprehensive, family-centered, coordinated, compassionate and culturally effective –Include young adults in decision-making at the earliest age possible
