The wellbeing of young people is at the heart of the transition process

2. What is Meant by Transitions?
Transitions is the period of time during which a young person moves towards becoming an adult.
If a young person has care and support needs it may be the responsibility of Children’s Services to provide social care support before they are 18 years old
When a young person becomes 18 years old, this responsibility may transfer to Adult Social Care
The Transition Team work with young people through this time of change

4. A Transitions Assessment
This should include:
The impact of the needs that a young person may have (for care and support) on their well being
The outcomes a young person wishes to achieve
Whether and to what extent the provision of care and support could contribute to the achievement of these outcomes

5. The Transitions Assessment
Should…
Involve the young person, their parents / guardians and any other person they would like to be involved
Be carried out in a reasonable timescale
Consider short term outcomes as well as medium and long term outcomes
Support the young person and their family to plan for the future
Build on existing arrangements
Take place at the right time for the young person
Happen early enough to allow for planning
Consider how interventions other than care and support could contribute to the young person’s desired outcomes

6. Care Act Eligibility Criteria
There is a 3 part test to decide whether a young person meets the eligibility criteria under The Care Act.
Are the adults needs caused by a physical or mental impairment or illness?

7. Care Act Eligibility Criteria
As a result of the adults needs they are unable to achieve two or more specified outcomes?
An adult is to be regarded as being unable to achieve an outcome if the adult:
is unable to achieve it without assistance;
is able to achieve it without assistance but doing so causes the adult significant pain, distress or anxiety;
is able to achieve it without assistance but doing so endangers or is likely to endanger the health or safety of the adult, or of others; or
is able to achieve it without assistance but takes significantly longer than would normally be expected

As a result of the adults needs they are unable to achieve two or more specified outcomes?
Managing and maintaining nutrition
Maintaining personal hygiene
Managing toilet needs
Being appropriately clothed
Being able to make use of the home safely
Maintaining a habitable home environment
Developing and maintaining family or other personal relationships
Accessing and engaging in work, training, education or volunteering
Making use of necessary facilities or services in the local community including public transport and recreational facilities or services
Carrying out any caring responsibilities the adult has for a child

9. The Care Act Criteria And
As a consequence there is or is likely to be a Significant impact on the person’s well-being

10. Resource Allocation Tool
This is a tool which is used to determine an indicative figure for a Personal Budget.
A figure is reached by asking a number of questions relating to a young person’s abilities, needs and the level of support they may be able to access.

12. Support Plan
A Support Plan will be designed with a young person and their family or carers to decide how the young person’s needs and outcomes will be met.
For example:
Outcome
I would like help to prepare for employment and be suppprted to find a job
What will happen and when
A referral will be made to supported employment eg. Building Bridges

13. Support Plan There are lots of ways to arrange for care and support.
For example:
Direct Payments - employing and arranging your own support
Direct Services – having support arranged for you.
Voluntary Services
Private arrangements
