Provide direct services to enable seniors to remain in their own homes with the same or improved quality of life for as long as possible; increase physical activity and improve nutrition in youth with the purpose of reducing obesity; and improve access to primary and preventive health care. 3

Grant activities will: increase seniors ability to remain in their own homes with the same or improved quality of life for as long as possible, increase physical activity and improve nutrition in youth with the purpose of reducing childhood obesity, and improve access to primary and preventive health care for communities served by CNCS-supported programs (access to health care). 4

National Performance Measures are organized into five Tiers Priority Measures (Tiers 1 & 2) roll up into the CNCS strategic plan Pilot Measures (Tier 3) are additional national performance measures in the Focus Areas Tiers 4 and 5 are for applicants not using national measures Healthy Futures Measures are in Tiers 2 and 3 5

Homebound or Older Adults and Disabled Individuals H8: Number of homebound OR older adults and individuals with disabilities receiving food, transportation, or other services that allow them to live independently. H9: Number of homebound OR older adults and individuals with disabilities who reported having increased social ties/perceived social support. 6

Increasing Access to Nutritious Food H10: Number of individuals receiving emergency food from food banks, food pantries, or other non-profit organizations. H11: Number of individuals receiving support, services, education and/or referrals to alleviate long-term hunger. H12: Number of individuals that reported increased food security of themselves and their children (household food security) as a result of CNCS-supported services. 7

If an applicant has a program model focused on increasing the social ties of homebound or older adults, or individuals with disabilities, they MUST select from the priority measures provided. 8

Access to Care H1: Number of individuals who are uninsured, economically disadvantaged, medically underserved, or living in rural areas utilizing preventive and primary health care services and programs. H2: Number of clients to whom information on health insurance, health care access and health benefits programs is delivered. H3: Number of clients enrolled in health insurance, health services, and health benefits programs. H4: Number of clients participating in health education programs. H7: Number of clients receiving language translation services at clinics and in emergency rooms. 9

Reducing Childhood Obesity H5: Number of children and youth engaged in in-school or afterschool physical education activities with the purpose of reducing childhood obesity. H6: Number of children and youth receiving nutrition education with the purpose of reducing childhood obesity. 10

In addition to documenting that the need exists, programs must describe how the interventions the AmeriCorps members and volunteers are engaged in are both evidence-based and will have a measureable impact. The term evidence-based refers to data that supports the presence of a cause and effect relationship between the intervention and the outcome. The evidence provided must support the programs theory of change, which articulates why the intervention (member and volunteer activities) will lead to the outcomes identified by the program. 11

Sources of Evidence May Include: Past performance measurement data Results from an impact evaluation of the program Research studies documenting the outcomes of similar programs Evaluations documenting the outcomes of similar programs 12

Programs are required to have at least one aligned performance measure for their primary service activity. Aligned measures have two key components: Output and Outcome Alignment refers to whether: The outcome is likely to result from the intervention and output, and Whether the output and outcome measure the same population. 13

Outputs are counts of the amount of service that members or volunteers have completed. They do not provide information on benefits to or other changes in the lives of members and/or beneficiaries. 15

Outcomes specify changes that have occurred in the lives of members and/or beneficiaries. They should be: Realistic Measurable during grant period Relevant to theory of change 16

Outcomes measure changes in: Attitude Behavior Condition Most programs should aim to measure a quantifiable change in behavior or condition 17

Outputs H1, H2, H3, H4, H5, H6 and H7 do not have national outcomes. For these outputs, applicants and grantees will be required to develop their own outcome that is aligned with the selected national output. 18

Children in the community have higher rates of obesity than the national average. Community Need Number of children demonstrating measurable gains in aerobic fitness, an indicator of improved health and decreased risk for health issues such as obesity. Intermediate Outcome Number of children participating in structured physical activity program. Output AmeriCorps members will engage elementary school children in a structured program of physical activity 5 days per week. Member Activity 19
