Clinical Resource Management Inpatient Care Coordination 2002

Medicine Clinical Care Coordinators There is an RN Assigned to each service and specialty. The following slides outline what the RNs assigned to each specialty do.

Care Coordination Role Daily collaborative interactions with physicians, nurses and ancillary staff to coordinate/facilitate the delivery of high quality, individualized, cost-effective patient care. Attend Daily Rounds Assist with expediting priority services to facilitate medical clinical decision- making Functions as a resource: – Clinical documentation to support medical necessity of services being provided – Expected LOS – Identification of potential discharge planning needs – Quality care/risk management issues – Interpretation of federal regulations as they apply to clinical situations. Assist & support the discharge planning process.

Care Coordination Role Analyzes patterns and trends in patient care for quality improvement opportunities Identify & monitor resource utilization, process & system delay Report results to appropriate designated departments/committees with recommendation for performance improvements
