Care Delivery System

2 Professional Practice Model (PPM)
What is a professional practice model (PPM)? A PPM is a system (structure, process and values) that supports registered nurse control over the delivery of nursing care and the environment in which care is received. A PPM is a schematic description of how nurses practice, collaborate, communicate and develop professionally to deliver the highest quality of care for those served by MUHC (AN CC, 2008). A PPM has five sub-systems: values, professional relationships, patient-care delivery model, management approach and compensation and rewards Professional values expressed in PPMs include nurse autonomy, nurse accountability, professional development and an emphasis on quality care. Teamwork, collaboration and consultation are approaches to enhance professional relationships. Why is it important to have a PPM? A PPM defines the components of a unified nursing practice throughout MUHC. Each PPM component depicts how nurses collaborate, communicate and develop professionally.

3 Care Delivery System (CDS)
What is a care delivery system (CDS)? A system for the provision of care that delineates the nurses’ authority and accountability for clinical decision-making and outcomes. The care delivery system is integrated with the practice model and promotes continuous, consistent, efficient, and accountable nursing care. The care delivery system is adapted to regulatory considerations and describes the context of care, the manner in which care is delivered, skill set required, and expected outcomes of care.

5 Nurses Caring for our Patients and Families
The professional nurse at University of Missouri Health Care is the core collaborator who directs communication within the healthcare system, advocating for patients and their families throughout the continuum of care. Nurses Caring for Our Patients and Families is accomplished through our Care Delivery Model.

6 CDS University of Missouri Health Care’s Nursing Care Delivery System is based on the belief that the professional registered nurse assists individuals to achieve their optimal self-care level. The registered nurse establishes a relationship with individual patients and families, and has responsibility, authority, and accountability for patient care coordination in collaboration with other health care professionals. The CDS ensures that a Registered Nurse: Is accountable and assigned shift responsibility for each patient. Defines, directs, and evaluates all patient care Supervises delegated patient care activities Ensures patient care needs are met Coordinates staff competency with patient care needs Provides patient assessment on admission, during hospitalization, transfer, and at discharge Coordinates patient/family education specific to identified health care needs; directs patient specific emergencies Ensures infection control and safety precautions are maintained

The professional nurse actively participates in shared decision making representing their specialty and the profession.
