The Team The team consists of dedicated individuals comprising of: Trusties Doctors Nurses Managers Health Professionals (Physiotherapists and Nutritionists) And community volunteers.

Aims & Objectives To work in partnership with local charities and Health providers to reduce the burden of disease within the community. To educate and motivate the community in better health education and lifestyle choices. Reduce the impact of an increasing population on NHS resources. Meet Health inequalities
Services Regular sessions within the community providing: Basic Health Checks – Height, Weight, BMI, Blood Sugar, etc Management of High Blood pressure Control of Diabetes Obesity Management Stress Management Dietary and Physical activity advice
Outcomes Expansion of the project to different areas and faith groups. Increase in promotion and uptake by the public A reduction of high BP for those that attend regularly Greater understanding of Disease Management Increased understanding of Lifestyle choice and effects An alternative resource in out of hours services An access channel for people not registered with a GP
Case Study 1 Patient was very depressed because of her weight, & high B.P. as she is getting married. She is regularly attends sessions at SHIP for the last 1month. She had lost 3kg since attending By adopting healthy eating, she has stopped skipping meals. The changes have made her more relaxed & increased her confidence. Her B.P. came down from 140/95 to 120/80, BMI from 25 –23 and reduced her waist from 30” to 27”.
Case Study 2 Mr.A suffers from numbness in his fingers. He never checked his Blood Sugar or Blood pressure and he is overweight, with BMI of 23. He is not registered with any GP. He suffers from Diabetes with reading of 12.1mmol/l. Since coming to SHIP regularly, he had lost 4kg in 2mth, his blood sugar is 6.1 mmol/l at the moment.
