2 Background The South Pembrokeshire Cluster identified high prevalence around Cardiac/Heart Failure and they decided to think outside the box and look at an holistic, behaviour change intervention to improve health.  The vision was supported by the Public Health Team and an opportunity was created for an individual to deliver prudent, preventative, healthcare for a cluster of GP practices in the South Pembrokeshire locality.  The vision was to support the patient (and their family where appropriate) to make positive and sustained changes towards healthier lifestyle choices. Additional support being provided, to patients, around improving their physical health, activity levels and mental well being in order to support healthy ageing. 8 Case Study GENDER: Male AGE: 55yrs ISSUES IDENTIFIED AT IA:
ISSUES IDENTIFIED AT IA: BP 152/105 – referred to nurse, nurse referred on to GP Weight – 115kg BMI – 35.8 Low mood Moderately Inactive - Patient was generally moving around during the day but very slowly Feels out of breathe when performing minor tasks Poor eating habits caused weight gain Excessive sugar consumption GOALS SET WITH PATIENT: Reduce sugar consumption Read food labels Understand portion sizes Increase intensity of current activity levels    GOIALS ACHIEVED: BP 119/80 with no medication Weight – 98kg BMI – 30.5 Has increased physical activity during his normal day No longer feels out of breathe unless walking up hill Eating regular healthy meals Understands food labels to inform snack choices PATIENT COMMENT: “I can’t believe that I have lost so much weight and my blood pressure has come down just by changing my diet and being more active” Helen Heaton – Healthy Lifestyle Advisor South Pembrokeshire GP Surgeries –June 2017
