18 Fit & Active Barnet Campaign
Aim – localised campaign launched in July 2018, encouraging and inspiring residents to lead more active and healthier lifestyles Addresses barriers to participation and communicates the health and wellbeing benefits associated with being active Fit & Active Barnet Hub will support the participation journey, providing a ‘one stop shop’ for all things SPA related ie: Fit & Active Barnet Card registration Activity finder App News, tips and case studies Volunteering opportunities

19 What is Healthwise? Model for physical activity interventions covering wide range of medical conditions, compliant with NICE guidelines. Healthwise operates within 21 Local Authorities. Tailors programmes to meet local needs and address health priorities Introduces patients to a low cost physical activity programme Offers a menu of established evidence based programme options including physical activity on referral, cardiac referral and weight management. Provides supervised or independent pathways, delivered by highly trained staff. Partnership work with extended services including dietetics, cardiac rehab, pulmonary rehab.

22 Healthwise Physical Activity Referral Scheme (PARS)
The Healthwise Physical Activity referral scheme introduces individuals to the benefits of physical activity in order to help reduce risk factors for chronic disease and manage existing medical conditions. 12 week, low cost physical activity programme Helps to become and remain more physically active Increase fitness levels Increase knowledge, skills and confidence Improve health and well-being

23 Barnet Healthwise PARS: Who is the scheme for?
Barnet residents or patients from a Barnet based GP Over 16 Medical condition meets inclusion criteria – existing condition or at risk of developing condition. Ready to make a change (pre-contemplation – contemplation phase) Physically inactive.

24 Referral Common reasons for referral: High BP, diabetes, obesity, asthma, circulatory/resp diseases, depression/anxiety, arthritis… See referral form for full list of conditions Only access is via GP referral GP should give details of the Healthwise Coordinator to patient at time of referral GP must be registered with the scheme (can be registered via the Healthwise Coordinator at local ‘Better Leisure Centre’ (Barnet Copthall) cepn.barnetccg.nhs.uk/Downloads/HEALTHWISE%20REFERRAL%20FORM.docx Current waiting time: 3 weeks Programme available at Copthall Leisure Centre and Burnt Oak Leisure Centre.

25 The healthwise team will:
Perform an initial assessment and prescribe suitable physical activity (supervised or non-supervised) that can be carried out over 12 weeks Set up with Healthy Living membership which gives unlimited access to gym, swimming and group exercise classes Assess and monitor progress – reviews at 6 and 12 weeks Motivate and encourage during activity programme Advise on how to exercise safely and with confidence Answer questions

26 Cost Better Healthwise membership is an inclusive membership which gives access to the gym, swimming and group exercise classes based on what is suitable for individual needs and medical condition. Payment is available in three options: • An annual membership of  £ • A monthly Direct Debit for £13.20 per month for the first year. • A Pay & Play membership of £3 per visit
