Do now Distribute some of the material around nutrition to carers of people with dementia in Purbeck. Also spread the word to team members. Support and encourage POPP workers to promote and take an active role in delivery I will make GPs in Dorset aware of the project and ask them to consider malnutrition within their new project and CCG funding for the over 75’s Get day centres to start carrying out screening to follow up on the work of the pilot Contact Area Manager about joint working with other housing associations to promote the programme to tenants

Do soon Look at extending the Reach Out service that Age UK provides in the Dorchester to Purbeck. This service helps people who live alone to access groups like lunch clubs. Help to develop content on nutrition and hydration for upcoming community channel (online) Make sure all the hospital nurse leaders are aware of the project and encourage them to work with the community to improve discharges and sharing of information. Signpost people to Help and Care. Raise awareness of project at every opportunity. Share the word with colleagues. Initiate a weigh in for those that want to keep a healthy weight at Sheltered Housing Schemes alongside coffee mornings.
