LASER and UV LIGHT SAFETY University of Notre Dame

ADMINISTRATIVE POINTS All Class IIIB and Class IV lasers must be registered with, and evaluated for hazard by, Risk Management and Safety. All users of Class IIIB and Class IV lasers, and UV light sources, must complete safety training. Documentation of this training must be present in the lab. Class IIIB and Class IV laser users must complete a baseline eye exam through Grossnickle Eye Center prior to using such units.

ADMINISTRATIVE POINTS (con’t.) A written Standard Operating Procedure must be present and available for each Class IIIB and Class IV laser in the lab. A Laser Safety Manual must be present in each lab housing a Class IIIB and/or Class IV laser.

SIGNAGE Proper signage must be present at the entrance to the Hazard Zone. A lighted warning sign must be in operation every time a Class IV open-beam laser is in use.

LASER SET-UP All reflective not specific to the project must be removed from the Hazard Zone. This includes watches, rings, and other jewelry worn by the laser users. The beam must never be at eye level, either seated or standing, for any personnel in the lab. Eye wear of proper optical density must be worn in the Hazard Zone at all times that the unit is operating in open-Beam mode.

ULTRAVIOLET LIGHT SOURCES UV equipment must have warning labels affixed. UV light sources must be properly shielded.
