Where we wereWhere we are nowWhere we will be

Where We Were

Moffat Project 2008 – 2010 Partnership with carers centres and NHS 4 main care groups on 9 hospital sites Aim : to identify carers in the hospital setting, involvement of carers in discharge, improving support for carers. Carer Awareness Training to staff

Where We Were What we learned System very dependant on visible presence to generate response – not sustainable Need for carer identification to be part of patient pathway Identification of carer alone not enough to provide good outcome for carers Training needs to directly link to organisational priorities and outcomes

Where we are now

Where We Are Now Toolkit for training incorporating support available to carers and how to access Identifying opportunities for workforce development Responding to Directorate priorities Exploring systems to improve identification of carers Working in partnership on training priorities

Carers Information Strategy Dementia Strategy Older People in Acute Care Person Centred Framework Corporate Priority – Carers to be identified and supported

“Getting to know me” document “ Are you a carer?” question in Nursing Admission document Scottish Patient Safety Programme Power of Attorney What matters to me – who matters to me ? Anticipatory Care Planning Discharge Planning
