 A multidisciplinary longitudinal experience for MS3 and MS4 students  Twice per month clinics  Care of patients with 3+ chronic diseases  Each student has his or her own panel of patients

 Teams of medical, pharmacy and nursing students  6 faculty supervise 8 teams (each with 5-6 students) twice per month  23 4hrs X 8 = 704 faculty hrs

 All MS3 and MS4 students participate in twice/month longitudinal clinics in care for chronically ill patients who are living in poverty;  At quarterly retreats, students reflect upon the processes of care in their longitudinal clinic. This resulted in Diabetes Day and Prevention Day Prior to Prevention Day, each MS3 reviewed the chart of patients in panel and determined which services were needed; Another creative component of Prevention Day was individualized nutrition counseling provided by nursing students.

 Students learned about and applied techniques to improve clinic work flow, implementing value stream mapping, status boards, and check-out logs.  Retreat also resulted in a new process for transitioning care from the MS3 Primary Care Provider (PCP) to the new PCP. Every MS3 reviewed the care of each patient delivered throughout the year, identified any gaps in care, and wrote a summary with instructions for the incoming MS3.  Two teams piloted ePrescribing. Its success allows ePrescribing implementation throughout the clinic.

 Develop Student Academy for MS4 students  Quarterly retreats for Longitudinal clinic will focus on clinical performance outcomes.  Students will identify areas of clinical performance which require improvement and will develop and implement rapid-cycle improvement plans.  Student performance expectations within the Community Health curriculum, integrating clinical skills instruction with clinical experiences in Bedlam-L and Bedlam-E.
