In 1972, under the sponsorship of the late Congressman F. Edward Hébert, Congress enacted the Uniformed Services Health Professions Revitalization Act Public Law The AFHPSP was created under authority of the Uniformed Services Health Professions Revitalization Act of 1972, and is governed by Department of Defense Directive (February 1975) and SECNAVINST A (November 1987). The program’s purpose is to ensure adequate numbers of commissioned officers on active duty who are qualified in the various health professions. F. Health Professions Scholarship Program SCHOLARSHIP ELGIBILITY Be a citizen of the United States or Permanent Resident Be enrolled in or have a letter of acceptance to an accredited dental school in the United States or Puerto Rico Meet eligibility criteria for appointment as a commissioned officer in the U.S. Army Reserve Maintain full-time student status during the entire length of the program

Health Professions Scholarship Program Professional Status Dental Length of Assignments to AD: This is determined by the time in the HPSP & the time in Advanced General Dentistry. General rule is one year per year of sponsorship in the HPSP with a minimum of three years of service. Assignments to AD: Each Doctor will fill out a DA form 160-R, with their station choices, IAW AR , chapter Special Pays & Continuous Medical Education: Each specialty has a specific group of special pays and the Army gives the opportunity to attend many conferences and trainings to improve each doctor’s performance. Active Duty Service as CPT Dental Corps

Health Professions Scholarship Program Special Cases Dental School Academic Failure - Serve as a Dental Officer for the time equal to the sponsorship in the rank of CPT. - Serve as a MS Officer for a time equal to the sponsorship “up to date”(3 years minimum). Advanced General Dentistry Program - If selected for AGD, you will train at one of these Army health care facilities: Fort Lewis, Washington Fort Campbell, Kentucky Fort Carson, Colorado Fort Benning, Georgia Fort Jackson, South Carolina Fort Meade, Maryland Fort Sill, Oklahoma Baumholder, Germany

Active Duty Pay - Commission as a Captain: CPT’s AD Pay: $3,221 Basic Allowance for Housing (BAH): Non taxable Basic Allowance for Subsistence (BAS): Non taxable Special Pays (if you qualify) Variable Special Pay - paid monthly and based on creditable service / rates ranging from $3,000 to $12,000 Board Certification Pay - paid monthly for Dental Corps officers who are board certified and based on creditable service / annual rates range from $2,500 and $6,000 Additional Special Pay - for Dental Corps officers who accept a 1-year extension to their current military obligation / annual rates range from $4,000 to $15,000
