Interacitve workshop: Person Centred Planning

Person Centred Planning What we believe and think about people with developmental disabilities shapes the oppurtunities we help people with disabilities to develop and the way we provide the assistance they need. Take a look at the copy Stop assuming, „It can‘t be.“ Start asking, „What would it take?“

beliefs - oppurtunities - experiences

Find Direction It`s easy for people with disabilities to get trapped in low expectations and have to settle for second class citizenship. It helps to have a way to think about what the good things in life are for any person

Enable valued experiences Belonging Being respected Sharing ordinary places Contributing choosing

How will an idea for making things better increase valued experiences?

Someone is missing....Working together!

Person Centred Planning “How can we make sure that support services respond to the individual needs of persons with disabilities and help them live included in the community?“

WHY Person centred planning? Persons with disabilities have the RIGHT to person centred support that enables them to live in the community. The European Union and most Member States ratified the United Nations Convention on the Rights of Persons with Disabilities. In this way, they committed to making sure that persons with disabilities can get the support they need to live independently and take part in the community. A high proportion of disability services are still provided on an institutional basis, in care homes which are isolated from the community. The transition from institutional to community-based care requires organisations and carers to re-think the way support services for persons with disabilities are designed, organised and delivered.

PCP has 5 key features 1.The person is at the centre one page profil, flowertemplate, arrowtemplate 2.Family members and friends are partners in planning social network, circle of friends 3.The plan reflects what is important to the person (now or for the future), their capacities, and what support they require My review booklet, school booklet

PCP has 5 Key feature 4.The plan helps build the person’s place in the community and helps the community to welcome them. It is not just about services, and reflects what is possible, not just what is available. Maps and Path 5.The plan results in ongoing listening, learning, and further action. Putting the plan into action helps the person to achieve what they want out of life.
