Patient Involvement: Family Health Teams How did the patient organization contribute: –Collaborates in identifying and training patient leaders and professional coaches in self-management; –Development of self-management tools to be used in collaboration between patients and healthcare providers; –Training of FHT staff on self-management support skills (e.g., asking open-ended questions, listening, and identifying barriers) –Engaging community-based support services (nutrition, fitness, stress management, and addictions counseling) in self-management model and collaborating on protocols for referral; –Evaluation and feedback on self-management partnership. Outcomes: Measures have been developed to measure changes in patient and health care professional behaviour including the measures that had been identified by the Ontario Ministry of Health as relevant to their Chronic Disease Management programs. Promoting patient-centred healthcare around the world Conclusions The strain on healthcare systems is a massive global problem Meaningful patient engagement must be central to efforts to refocus health systems on people and patients. Issues of access to healthcare are integrally linked with provision of information and patient involvement in the delivery of healthcare and development of health policies The best way to achieve patient-centred healthcare is to have an approach based on strong values of respect, partnership and equity – no patient-centred healthcare without cooperation of all stakeholders IAPO welcomes and supports initiatives to reorient the healthcare system to be patient-centred
