Research Evidence – There is growing evidence of the benefits of its practice (including patient quality of life, improving physiologic measures and increasing efficiency of use of healthcare services) See ‘What is Patient-Centred Healthcare?: A Review of Definitions and Principles’ Available online: Patients support for a patient-centred approach – An IAPO study into the perceptions of healthcare of patient group members was undertaken by Consensus Research and supported by Pfizer Inc. The 12 countries included in the study were: UK, Germany, France, Hungary, Austria, Czech Republic, Italy, Spain, Belgium, Sweden, Canada, Nigeria.

Promoting patient-centred healthcare around the world The study results: demonstrated strongly shared views on the needs and concerns of members related to currently administered healthcare signalled a need for a shift to a more patient-centred approach to healthcare identified three recurring themes that illustrate shared concerns related to: timely access to the best treatment and information the right to participate in decisions at the individual patient level patient involvement in policy-making

Patient Involvement: Family Health Teams Partners: Institute for Optimizing Healthcare Outcomes (IOHO) – Blue Sky Family Health Team Objective: To improve patient self-management and self- management support within the context of regional healthcare reform Why: Chronic Disease Management in Canada was not meeting the needs of patients How: Through an integrated structure of healthcare providers, including physicians, nurse practitioners, pharmacists etc, and patient representatives. Patient Involvement: Family Health Teams How did the patient organization contribute: –Collaborates in identifying and training patient leaders and professional coaches in self-management; –Development of self-management tools to be used in collaboration between patients and healthcare providers; –Training of FHT staff on self-management support skills (e.g., asking open-ended questions, listening, and identifying barriers) –Engaging community-based support services (nutrition, fitness, stress management, and addictions counseling) in self-management model and collaborating on protocols for referral; –Evaluation and feedback on self-management partnership. Outcomes: Measures have been developed to measure changes in patient and health care professional behaviour including the measures that had been identified by the Ontario Ministry of Health as relevant to their Chronic Disease Management programs. People at the Centre of Health Care initiative: World Health Organization – Western Pacific Regional Office Identified changes in the delivery of healthcare globally that are altering community expectations of health care delivery. A common vision and framework for policy and action A system wide approach across four domains: Individuals, families and communities Health practitioners Health care organizations Health systems Within each of these domains, a policy framework has been designed as: “a menu of evidence-based interventions and best practices that promote people- centred healthcare and facilitate policy development and implementation at country level.” Promoting patient-centred healthcare around the world Patient-Centred Healthcare: A system wide approach

Promoting patient-centred healthcare around the world Conclusions The strain on healthcare systems is a massive global problem Meaningful patient engagement must be central to efforts to refocus health systems on people and patients. Issues of access to healthcare are integrally linked with provision of information and patient involvement in the delivery of healthcare and development of health policies The best way to achieve patient-centred healthcare is to have an approach based on strong values of respect, partnership and equity – no patient-centred healthcare without cooperation of all stakeholders IAPO welcomes and supports initiatives to reorient the healthcare system to be patient-centred
