Falls and Fractures Alliance Formed by NOS & Age UK in 2012 Board (meets 3 times a year) evolved from the DH Fragility Fracture Programme Board Alliance (meets once a year) brings organisations (around 23) together to try to reduce falls & fractures, specifically hospital admissions for hips # in >65yrs Members sign up to Declaration and provide Action Plan

Providing members with overview of national policy in falls & fractures and identifying gaps/changes to be made Working together with new NCDs Developing ideal, integrated care pathways for falls & fractures Winter planning 2014 Consultations including CQC Patient experience – NHFD reporting FFA Current Areas of Work

Proven delivery model for secondary fracture prevention: Fracture Liaison Service FLS Database, part of FFFAP FLS recognised as best practice FLS endorsed by DH and Care Minister April meeting with NHSE to inform strategic direction of secondary fracture prevention Secondary fracture prevention
