SUMMARY Some indicators of the level of CB in Kent What happens at the moment? Is there a strategy? What do we need to do The idea of the Managed Practitioner Network

Who is challenging depends how well we understand what the person needs, and how well we provide this what about people with autism, complex physical or mental health needs? What about people who don’t cause problems but who do not appear to thrive?

Some Indicators: District estimates of people at risk of breakdown = West Kent Districts place : 101 people Out of County 50 people in East Kent 54 people in Mid Kent average 17 in other West Kent Districts range from 9-50 in own District (total 136) + NHS placements

Some Indicators cont. West Kent NHS placements = 450 –?60 have very challenging needs East Kent NHS placements = 320 –? 50 have very challenging needs people placed in Kent from elsewhere –? How many have very challenging needs

What happens at the moment? Some specialist services –in county & out of county some staff with special skills –statutory agencies + independent struggles about the money eligibility + crisis response

So what do we need? Coherent approaches to commissioning what sort of support / provision is wanted? individually planned approaches that develop with the person mixed economy collaboration resources being spent well outcomes / VFM / reinvestment

Coherent approaches to practice value specialist knowledge and skills be pragmatic about the reality and extent of people’s needs, and what they can provide promote independence invest in comprehensive systems Early Identification Early Intervention Crisis management Long term support
