Comp for Injury - APMC Form FS (authorized by COMP) Limit to First Aid or “Camp Crud” Use when: 1) No further medical treatment is necessary 2) No lost time due to injury/illness 3) Initial treatment did not involve surgery or hospitalization A CA-1 (injury) or CA-2 (illness) is still completed

ASC & Serious Injuries During business hours (M-F MST) - call Option 2 for HRM Workers’ Compensation Group to request authorization for immediate medical care Outside business hours – COMP has authority to give verbal authorization to hospital for injuries (not illness-DOL needs to determine based on med doc)

CA-16 Authorization for Examination and/or Treatment Can be issued within 7 days of injury Will not be issued without approval
