Phone: (817) 408-4850 Issued Materials: Shirt and Shorts Extra Materials: Sweats/Wind Pants-BLACK OR GRAY, Tennis Shoes and a Combination Lock. *NO LOCK, NO LOCKER!!!! Replacement Policy: The student will be issued a school owned Physical Education uniform, which includes a shirt and shorts. If the items are damaged or lost, the replacement cost is $5.00 for the shirt and $11.00 for the shorts. The student is responsible for this charge and will not be issued a replacement shirt/shorts until the fee is collected. This could result in a deduction of class points/office referral due to not suiting out. Grading: Suiting Out/Participation: 90% *20 pts. Daily=100 pts. Weekly 10 pts. -Appropriate Dress (shirt, shorts, and tennis shoes) 10pts. -Participation 2nd Time NOT Suiting Out will result in parent phone call. 3rd Time NOT Suiting Out will result in office referral/lunch D-hall. 4th Time NOT Suiting Out will result in office referral/PM Tutorial. 5th Time NOT Suiting Out will result in office referral/2 PM Tutorials. 6th Time NOT Suiting Out will result in office referral/Rise. 7th Time NOT Suiting Out will result in office referral/Rise. Written Tests: 10% *Written Tests will consist of weekly journal entries. Dressing Room Rules: *No dressing in the shower or bathroom stalls *Dressing room will be kept clean and orderly *Horseplay will not be tolerated Tardy Policy: The student needs to be in his/her P.E. roll call line/bleachers PRIOR to the tardy bell. Each student will be given 3 minutes to dress in P.E. clothes and 5 minutes to dress back into school clothes at the end of the period. Our goal at this grade level will be to stress the importance of participation in physical education both in and out of school. As a department we want each student to have the opportunity to acquire knowledge of games and build for the enjoyment of physical activity, which will lead to the learning of many life-long skills. Student Behaviors Be Prompt Do NOT ask to leave the gym without your agenda. No agenda = No exit Once a student enters a gym, he/she will not be allowed to leave unless it is an emergency or unless they are called to the office.

Students must use the restroom during the five minute passing period and should not be late to class. It is NOT allowable to come to class and ask permission to be late so you can use the restroom. Take care of business during your five minute passing period.

Student Behaviors Insubordination: Students must courteously and respectfully comply with the directives of any staff member at all times. Failure to comply with the directives of school personnel acting in the performance of their duties will be considered insubordination and subject to immediate disciplinary action (referral).

Make-Up Work Students will normally have one day for each day absent, plus one additional day to make up missed work. If the student knew of the assignment well in advance of the absence, it may be due immediately upon return as determined by the teacher. Teachers may also assign additional work to ensure students have sufficient opportunity to master the essential knowledge and skill or to meet course requirements. A student who does not make up assigned work within the time allotted by the teacher will be referred to the Z.A.P. Policy. Responsibility for requesting and completing make-up work rests entirely upon the student. It is your responsibility to remember to turn in assignments, and it is not your teacher's responsibility to remind you.

Parent-Student Communication AGENDAS AND ASSIGNMENTS Students are required to write down their daily assignments in their student agenda. Class time is given for this purpose. Parents should check their child's agenda on a daily or weekly basis for completion. Agendas will be checked regularly, and students will be held accountable for filling them out completely.
