Our Proposed Name for the East Midlands New Care Model IMPACT Improving Mental Health Pathways and Community Teams Having a positive IMPACT on our future.

Values HOPE We chose this because: Provides us with a purpose, allows us to believe things can be better. Offers options for a better quality of life

ONE TEAM Values Everybody’s contribution is equal and meaningful We chose this because: Everybody’s contribution is equal and meaningful We are all accountable and responsible We are all equal and involved

PEOPLE FIRST Values Everyone of us makes a difference We chose this because: Everyone of us makes a difference We should show compassion, empathy and respect We should respond to the needs of service users, families and staff The care delivered should be the best it can be

The important things to get right so you can live and work well with others in services Consistency To have our own space, where I can go and close the door Access to the world outside hospital Staff to react quickly if there are any incidents Let us take positive risks and learn from our own mistakes Give space when needed Involve people who are important to us.
