Policy Background and Guidance National Cancer Survivorship Initiative report 2013 Stratified Follow Up How To Guide 2013 PRINCIPLE 3: ‘Individuals should have a personal care record that includes a treatment summary’ National Cancer Strategy 2015 ‘Access to elements of the Recovery Package […] including a Treatment Summary’ Macmillan Treatment Summary How To Guide 2016 Long Term Plan 2019 - Personalised care

The Long Term Plan says “…every patient with cancer will get: a full assessment of their needs an individual care plan information and support for their wider health and wellbeing.” Also “patients will move to a follow-up pathway that suits their needs” How do End of Treatment Summaries (EOTS) fit into this?

Table Work (1) 1st discussion (10 mins) List out all the things that you think are preventing patients and GPs getting the full benefit of End of Treatment Summaries Choose the top 3 biggest barriers Choose those which you think NHSE could help with (whether in top 3 or not) Feedback to the room (5 mins) Consensus on the top 3 biggest barriers Consensus on what NHSE could help with

Table Work (2) 2nd discussion (15 mins) Thinking of the agreed top 3 biggest barriers How would you tackle them? What help do you need to tackle them? Feedback to the room (10 mins) Consensus on how to tackle these biggest barriers

Final question What progress do you think you can make on EOTS in your organisation in the next 1 to 2 years? Introduce EOTS for the first time? Increase the number of clinicians/MDTs using EOTS? Involve patients and GPs in local initiatives to maximise benefits of EOTS? Digital solution?
