Deductible & Prescription Claims & Balances Instructions In addition to signing and turning in the “Crum Trucking Health Reimbursement Account (HRA) Information and Claim Form” which you can print from our website or get from HR, you must pint off a recent copy of the amount that has been applied to your deductible thus far. If prescription drugs are part of your deductible, we will need to see your prescription Claims & Balances Sheet and the matching Prescription Receipts. Below are the instructions. Where To Start: Go to www.anthem.com and “Log in” or go to “Register Now” if you have not already registered. (“Member Log In” is on the top right corner of the page). WRITE DOWN YOUR USERNAME AND PASSWORD – Crum DOES NOT have access to this information if you lose or forget it. 1 2 For your Total Deductible Satisfied: 1. Click on the Claims tab 2. Under Claims click on Deductibles & Out of Pocket Amounts 3. Click Print Details (off to the right) 3 For your Prescriptions Claims & Balances: 1. From the home page when logged in - On the right hand side click on Prescription Benefits 2. Then towards the bottom under the section “Pharmacy Benefits” click on Claims & Balances 3. Make sure the Prescription claims & history Tab is selected 4. Adjust the date if you need to in order to reflect the current claims (Change Date Range) 5. Click on Print this Page 6. Bring these pages into HR with your signed HRA Request If you do NOT have internet access: Call the Customer Service # on the back of your Insurance card (800-295-4119) and ask them to mail you a copy of your Year to Date Summary showing your total Deductible Satisfied.
