Local Voluntary Partnerships (LVPs)

ONE CROYDON ALLIANCE In April 2017, the One Croydon Alliance was formed (for over 65s); to transform health and care in Croydon. The Alliance is a partnership between six organisations, namely; 1. Croydon Clinical Commissioning Group 2. Croydon Council 3. Croydon Health Services NHS Trust 4. South London and Maudsley NHS Trust 5. Croydon GP Collaborative 6. Age UK Croydon The programme has been planned as a 10 year, multiple work streams, health and care transformation plan.

VISION: Local Voluntary Partnerships (LVPs) The ultimate goals of the programme are better outcomes for people and the reduced demand on statutory health and care services. The driving force behind this programme is the belief that partnerships, in themselves, a positive force that would likely lead to better outcomes.

HOW LVPS WILL HELP DELIVER KEY OUTCOMES The 6 LVPs will be formed of local voluntary and community organisations coming together in an informal consortium representing each GP network. Mayday Network LVP Thornton Heath Network LVP East Croydon Network LVP Purley Network LVP New Addington and Selsdon Network LVP Woodside & Shirley Network LVP

HOW LVPS WILL HELP DELIVER KEY OUTCOMES All Croydon community VCS strategies could draw upon LVPs for key outcomes. In return LVPs will help inform and develop future strategic commissioning of Croydon Voluntary and community sector. Commissioning cycles that will be informed by the LVPs include: Community Fund (existing contract expires in Sept. 19) Prevention Fund (existing contract expires in March ’19)

Local Voluntary Partnerships- Update • LVPs are locality based partnerships between voluntary and community sector organisations to delivery defined outcomes each year • 6 x LVPs will be formed of delivery and non-delivery partners • LVPs will define the specific pilot/s under social isolation and primary care mental health support in each of the initial partnership bids • Delivery partners will be selected as required by the defined outcomes for 2018/19 – social isolation and mental health pilot/s • Non-delivery partners will be formed of infrastructure organisations

Local Voluntary Partnerships- Update • MH partner/s will be selected on the basis of organisations’ ability to deliver the Primary Care MH support pilot • LVPs will discuss and propose a single Directory of services, (subject to a e- market platform acquisition by the Council), and ensure that it is accessible to public • Locality based Point of Access & Information will be delivered in 2 phases. Phase 1 will be delivered through Advice Service Croydon Contract. Phase 2 will be delivered through LVPs proposals • Following local gap analysis, each LVP will deliver a proposal for 2019/20 delivery. Funding will be available through a streamlined grants process, subject to approval • LVPs will discuss the feasibility of a Community Exchange System in Croydon

Alignment with Social Prescribing Model Croydon has gained national recognition for it’s Social Prescribing model initiated in Thornton Heath CVA have established a strategic partnership to develop this model to work with the whole VS Each GP Network has or is about to initiate a Social Prescribing Board funded through GP Forward View One Croydon’s LVP Model is aligned with Croydon’s Social Prescribing Model

LVPs Governance One Croydon Transformation Board Together for Health and Care Programme Board will be responsible for monitoring and providing strategic direction to LVPs LVPs will provide progress reports to: Local Strategic Partnership Health and Well being Board - ICN Programme Delivery Group
