Social Prescribing Report

Hounslow Social Prescribing Service Quick Summary Self-care and prevention service transitioned into social prescription service managed by Self Care, Prevention and Volunteer co-Ordinator in July 2018 SPVC visited all GP surgeries to speak to practice managers and receptionist regarding new transition and system one requirement SPVC has recruited 15 new volunteers for the project SPVC Introduced new prescription sheet to all current and new volunteers The project is now live in all 47 GP surgeries. All volunteers attended the bimonthly training session organised by SPVC in which external service providers attended to speak about their services. (Mulberry centre, well-being network, COPD group, My independence and homelessness group) SPVC discussed how to use the PAMS sheet going forward within the wellbeing prescription form with volunteers SPVC has recruited 4 interns and 2 work experience placements to assist with the project. SPVC attended all locality meetings to discuss new prescription Volunteers have interacted with over 3191 patients and dedicated 1164 hours since the project has begun. SPVC has been invited to PPG meetings across the borough SPVC interacted with service providers where volunteers can signpost patients to. SPVC has ordered relevant information on winter conditions from service providers for the next few months Hounslow Social Prescribing Service

We aim to follow up with patients within 2 weeks of initial interaction with the patients consent. This service identifies community services and contacts within the borough that can help patients, patients often require additional information prior to what was discussed at our initial engagement  Social prescriptions to date – 152 Please find the subject areas volunteers have come across after initial engagement below: - Housing - Finance - Carer information - Domestic violence - Mental Health - Long term condition support 7% OF Patients followed up required more information on Housing problems. This ranges from problems arising from temporary accommodation to requiring bigger spaces for family and facing homelessness 14% required more help/information on financial circumstances. In most cases related to benefits 9% of patients were undeclared carers who needed initial information on services available to them both financially and emotionally 1 person so far has required help in relation to domestic violence 4% of patients required more information on mental health services 14% of patients with various long term conditions required more information on social activity to help Impact and Follow Up

We interacted with a total number of 152 patients A total of 246 Leaflet and information was given out to patients We have x2 volunteers fluent in Punjabi and Hindi who have successfully engaged with 26% of patients The majority of patients spoken to do not have enough confidence in the pharmacist and would prefer to go to their local GP The majority of patients spoken to require more information on Exercising and healthy eating. 62% of which are female 13% of patients spoken to have a Long-term Health condition with 40% of this being diabetes. Patients with diabetes seem to have been given medication for it but have no understanding or plan on how to self-care with the condition 15% of people spoken to have Asthma 6% of patients interacted with have lower back pain with no information on how to self-manage this We found 3% of patients have high blood pressure, with only 3% with high cholesterol problems We have found 6% of our patients have COPD. Our volunteers are now trained on different community services and self help services within the borough We have found the majority of mothers engaged with have attended first aid classes for babies and toddlers 70% of patients spoken to who need support with addiction are related to smoking We have found 15% of patients with long term conditions have completed the PAMS questionnaire 27 Patients were not willing to give their information to follow up on 55 Patients consented to giving their contact details so the volunteers can follow up 62% of patients spoken to found the information given at the first point of contact very useful 8 Patients spoken wanted more information The volunteers are coming across more patients with housing and financial needs Patient Representative Information - Spotlight Social Prescription Service
