Newly started anticoagulant medicine? Ask Your Pharmacist We are working with your community pharmacist. Please ask your pharmacist about your medicines. They will help you understand the best way to take your medicine and how to minimise side effects. Discussions with pharmacists about your medicines are free and have been shown to improve your care. It is a confidential conversation and will be provided in a private room within the pharmacy. This card is being used as part of a national programme to support patients needing review of medicines. We are working with the Academic Health Science Networks to achieve excellence in, and spread of, innovation for best practice.
