3 Why Community Pharmacy? Over 11,300 community pharmacies in the UK; 330 in Kent & Medway 1.6 million visits daily; 1.2 million for health-related reasons 99% of the population – even in the most deprived areas - can access a pharmacy within 20 minutes by car; 96% by walking or by public transport Pharmacist always on site Highly trained: 4 years at University + 1 year practice [5 years!]

5 Why Review Medication? Medication is the most common healthcare intervention In 2011/12: 885 million Rx’s dispensed, costing £8bn However, £300-£600m is wasted! 30-50% of drugs are not taken as intended Many patients stop taking their medicines after 1 month 4-5% of hospital admissions are due to preventable adverse drug reactions 41% of patients receive little explanation of side effects What can we do?

7 Medicines Use Review (MUR) MUR’s are designed to “improve patients knowledge, concordance and use of medicines” A confidential discussion in a private consultation room lasting 20-30 minutes It complements the annual clinical review conducted by GP’s. The outputs are reported to GP’s. It establishes the patients actual use, understanding and experience of taking medicines resolving poor or ineffective use of medicines identifying side effects and drug interactions that may affect a patients willingness to take medicines

8 Medicines Use Reviews (MUR) Designed to improve a patient’s knowledge, understanding and use of their medicines Pharmacists will ensure that at least 50% of MURs are target to patients who: 1. Are taking a ‘high risk’ medicine: NSAID, Diuretics Antiplatelets Anticoagulants (including LMWH) 2. Recently discharged from hospital 3. Respiratory Diseases

9 New Medicines Service (NMS) Designed to provide early support to patients to maximise the benefits of the medication prescribed NMS is specifically for patients newly prescribed medicines in the following areas: 1. Asthma and COPD 2. Type 2 Diabetes 3. Hypertension 4. Antiplatelet / Anticoagulant Therapy Service is split into three stages 1. Patient Engagement 2. Intervention 3. Follow-up

10 How can I get the best from my Community Pharmacist? Their primary purpose is to attend to your needs…..so let them! They do not judge……….trust takes time to develop. Find a good one & stick with them……loyalty works both ways! Ask them to review your medicines at least once a year If you are having ANY issues with your medicines – tell them If you decide not to take something – tell them If you have bought something over-the-counter – tell them Only order what you need

11 The use of medicines is sub optimal. Community Pharmacy plays a significant role in supporting patients to get more from their medicines. Community Pharmacy play a significant role in supporting patients and the public with public health issues: “prevention is the new cure” Urgent and Emergency Care Review has recognised the role community pharmacy plays in terms of patients contact and support for out of hospital care. 15 Pharmacy LPN The Pharmacy LPN will help to promote the role of the pharmacy profession and encourage and support increased collaborative working across the various sectors of the profession. It will become the preferred organisation of choice for local commissioners requiring independent, high quality, “pharmacy” advice when developing and updating their commissioning intentions. The Pharmacy LPN will ensure that the contributions of pharmacists working locally across the healthcare sector are focussed on patient care through collaborative working with commissioners, providers and patients.

Is identifiable by the public and other healthcare professionals
